Caring for our Senior Australians

Community Vision offers a number of programmes tailored to meet the needs of aging Australians. These programmes include:

Community Aged Care Packages – coordinated packages of care that provide flexible services to aged people who have been assessed as being eligible for residential care but who choose to remain living at home. (Funded by Department of Health & Ageing).

Veterans’ Home Care Programme - (Assessment function) – provides home support services to Veterans who are eligible under the Department of Veterans’ Affairs funded programme.

Seniors Day Centre – This is located at the Woodvale Community Centre. Seniors enjoy social contact and take part in recreational activities while a range of support services are provided as required. This centre includes a support group for people living with dementia. (Funded by WA Dept. of Health)

Podiatry – centre-based clinics are held weekly at Woodvale and Whitfords on a fee-for-service basis. Community Vision also provides a home-visiting podiatry service for frail seniors who are unable to travel to a clinic. (This latter service is funded by WA Dept. of Health)

Volunteer Services – volunteers provide support and assistance for seniors and disabled people living in the community and accessing the day centre services and resources.

Home Support Services – provides assistance and support to seniors and people living with disabilities under the Home and Community Care Programme with home help, personal care, social support, shopping and respite. (Funded by the WA Dept. of Health)

Meals Service– there are options available for people unable to prepare their own meals including a home delivered meal service or assistance with meal preparation in their homes. (Funded by the WA Dept. of Health)

Client Support Services – Community Vision's Assessment Consultants receive referrals, assess, plan and coordinate services with and for clients. The ongoing support includes reviews to assess changing needs and problem solving service delivery issues.
