Community Aged Care PackagesCommunity Vision is funded by the Department of Health and Ageing to provide coordinated packages of care to people who have been assessed by an Aged Care Assessment Team (ACAT) as being eligible for residential care but who choose to remain living at home. A person’s General Practitioner (GP) can arrange an assessment with an ACAT through the local hospital. Clients of the service are assessed as being eligible for nursing and residential care. With a package of care, a range of services is available including:
