Home and Community Care (HACC)

The Community Vision Home and Community Care Programme is funded by W.A. Department of Health. This programme includes the following services:

Home Support Services
This includes domestic assistance, personal care, social support, respite and home and garden maintenance to older members of the community and people with disabilities.

Seniors Day Centre Services
The Centre is located at Woodvale Community Centre. Seniors enjoy social contact and take part in recreational activities. Transport to and from the Centre is available and meals are provided for attendees. The centre also provides services for people experiencing the onset of dementia.

Meals Service
Home delivered meals or assistance with meal preparation are options available to people who are unable to prepare their own.

Podiatry
This funded service is provided as a home-visiting service for seniors who are unable to travel to a clinic.

Volunteer Services

Volunteers provide support and assistance for aged and disabled people living in the community and attending the Day Centre.

Client Support Services

Community Vision's Service Assessment Consultants receive referrals, assess, plan and coordinate services with and for clients. The ongoing support includes reviews to assess changing needs and problem solving service delivery issues.
