MY VISION

Community Vision is approved and funded through the Disability Services Commission (DSC) to provide 2 streams of services within its MY VISION program. Funding for both program options is linked with individual participants and is fully portable across agencies. In most instances MY VISION funding is recurrent. The types of services we provide are:

1. MY VISION - ALTERNATIVES TO EMPLOYMENT (ATE) & POST SCHOOL OPTIONS (PSO)
  • Support, provision and facilitation of community inclusion / access, skill development and recreation to high school leavers who are, due to their level of disability, unable to access either fulltime work or study. The participants are individually approved and funded by DSC to receive services under the PSO or ATE funding stream.
2. MY VISION - INTENSIVE FAMILY SUPPORT OPTIONS (IFS)
  • Support, provision and facilitation of community inclusion/access skill development, respite and recreeation to individuals approved under the Combined Application round of funding to receive Intensive Family Support (IFS) funding. Community Vision is able to either manage IFS funds completely for families or share management with families.

What MY VISION, ATE & PSO Options Offer?

All of the programs offered in MY VISION are goal based. This means:
  • We consult with program participants and/or their family to identify their goals for both the short term and long term.
  • We identify particular milestones that when reached are indicative of the progress of the goals.
  • We develop programs and activities which identify particular ways of achieving the goals together with the individual and/or their family.
  • We link the individual with a Client Support Personnel who has similar interests and is aware of the resources' required for the participants to achieve their goals.
  • We evaluate the goals and milestones regularly and provide feedback to the individual and/or their family on a regular basis.
  • In addition to working in line with the disability standards, we ensure that activities provided take into consideration the following aspects:
    1. valued roles & individual leadership of the participant, regardless of their disability;
    2. inclusion into the community by fostering citizenship;
    3. empowerment;
    4. sustainable capacity.
To be eligible for MY VISION services participants must be:
  • Living with a disability (intellectual, physical, neurological, cognitive, sensory, or autism).
  • Able to meet the criteria for Disability Services Commission's ATE, PSO & IFS approval guidelines.
  • If seeking IFS services, participants are required to lodge an application under the Combined Applications Round.

Registration and costs

MY VISION participants wishing to receive services with MY VISION are required to follow the following process:
  • Firstly be pre-approved for services through the Disability Services Commission.
  • Nominate Community Vision as their service provider if their required services are related to ATE or PSO services and inform DSC of their decision.
  • If services are related to IFS, then they must inform Community Vision of the IFS pre-approval funding level. Community Vision will then submit a funding plan to the Commission with agreed start date for services to commence.
MY VISION participants incur a $1.00 per hr administration levy and $5.00 transport fee per day/session. Additional costs payable by participants are fees to enter venues or events and meal costs.
