Community Vision is an Assessment and Coordination Agency for the Veterans’ Home Care (VHC) Program.

What is Veterans’ Home Care

Veterans’ Home Care (VHC) is a Department of Veterans’ Affairs (DVA) program designed to assist veterans who need practical help to continue living independently in their own home.

VHC cannot meet complex or high-level care needs, please contact us on (08) 9301 8227 if you would like to know more.

How can Community Vision help me with Veterans’ Home Care?

We are an Assessment and Coordination Agency for the VHC program. This means we are a DVA programme that is responsible for assessing eligible Veterans War Widows and Widowers who wish to access the VHC services.

Assessments are straight forward, and we are happy to answer any questions you might have. We also recommend reading this factsheet.

Does Community Vision provide services directly?

We do not provide Veterans’ Home Care directly, we are responsible for assessing and coordinating services. However, we can offer home care services outside of the VHC program.

We understand that eligibility can seem quite complex. That’s why we’re here to help. Whether you are eligible for the VHC program, or a different funding source, we will find out for you. We can also assist you with applying and making sure that you are satisfied with the services you receive.

When you need help with a Veterans’ Home Care assessment, contact Community Vision on (08) 9301 8227. We’re here to help.
